In-Home Care

Our company is available to provide care for your loved while allowing them to stay in the home. Our caregivers are able to travel to the location to assist the client according to the care plan we have in place. We are able to complete a multitude of tasks to ensure that the client is thriving in the environment. We will work together with the client, the family, and the medical personnel to keep the client safe at all times adjusting the care plan as needed.

 

Activity Daily Log

Each of our clients retain an ADL at the home where all information pertaining to their care is recorded. We are able to ensure that they are provided with everything the care plan requires, and in a timely manner. The team will work together to provide all services including, but not limited to, personal care, PT / OT Exercise Compliance, transportation (non-medical), and more. Your loved one is going to receive the one-on-one care they deserve. 

 

3 Hours to 24/7

Our team is available on a temporary basis to a long-term care plan. We are able to provide assistance in the home, and at a facility as well. We will work with you to evaluate the level of care that is necessary to improve your loved one's quality of life. We have the ability to assist with a personalized plan that will not only be best for the client, but also assist the family for their needs too. We are able to set up a short-term care plan, or a long-term care plan, that can be readdressed as needed to continue to support the needs of the client.

Short-Term and Long-Term

Our Team has the ability to assist with everything from companionship, returning home from rehabilitation facility, and more. Our caregivers have worked with Physical Therapy and Occupational Therapy to ensure we are completing exercises assigned to the client correctly and safely. We have extensive knowledge and experience working with clients with Dementia and Alzheimer's and have worked with Hospice in providing End of Life Care.
